In addition to the standard OPT participation reporting you are required to do at each six-month reporting mark, you are required to submit the OPT self-evaluation from the Form I-983 STEM OPT Training Plan at the 12 and 24-month reporting marks. Department of Homeland Security has a Planning Tool for STEM OPT reporting on its Study in the States website. The completed self-evaluation must be submitted to OVIS at the 12-month mark following the start of the STEM OPT period, and the final, completed self-evaluation must be submitted at the end of the 24-month STEM OPT period, or when the STEM OPT employment ends, whichever is sooner.
